Audit: Treating T2DM with comorbidities using SGLT2i

Overview

Learn about the latest guidelines around use of SGLT2i for patients who have diabetes and other comorbidities. As part of this activity, there will be an emphasis on the application of these guidelines in practice. You will do this through an audit, facilitated by a combination of a purpose-built dashboard to review and recall patients, in-consult education prompts within your MedicalDirector Clinical software and reporting on key metrics to help you measure outcomes. 

Learning Outcomes

  1. Identify the rate of initiations of SGLT2i in their diabetic patients with comorbidities. 
  2. Define patients who would benefit from SGLT2i initiation. 
  3. Implement a recall strategy to initiate eligible patients on SGLT2i based on RACGP guidelines. 
  4. Measure post-intervention benefits of SGLT2i (via HbA1c for diabetic control, uACR and EUC for chronic kidney disease). 
  5. Develop a practice wide plan (to reuse in the future) to ensure SGLT2i are being initiated for the right patients in accordance with guidelines.
