Chronic Disease Management

Overview

Chronic Disease Management is handled via the CDM panel.

1.      Within the patient's record, locate and click    to call the Consult panel.

2.      Within the Consult panel, locate and click

3.      The CDM panel appears. For further instructions, see;

o      Care Plans

o      Cycle of Care

o      GP Mental Health Treatment Plan

Care Plans

1.      Within the patient's record, locate and click    to call the Consult panel.

2.      Within the Consult panel, locate and click

3.      The CDM panel appears.

4.      Select a Care Plan from those available. In our example, we have selected GPMP.

o      If a care plan has already been created, it will be displayed in the list. You can review it, or add a new plan.

 

5.      The care plan opens.

o      The existing Authority Information, Conditions, Health Summary, and Biometrics for the patient are displayed on the left-hand side. You can elect to include/exclude this information.

o      If you need to update this information, click

 

 

 

6.      Via the Conditions drop-down menu, select a condition you wish to add to the Care Plan. In our example, we have added Asthma.

 

o      Add more conditions as desired. Each additional condition will appear below the next. To edit a row of information, click within it to activate 'edit' mode. Make changes as desired. Click off the panel to accept the changes.

       The image below displays an example of edit-mode. Notice that each field of information becomes active, awaiting your input.

       Assign providers of the suggested needs. For example, we have indicated here that the need of 'Give a patient a better understanding of asthma' (in the Needs column) must be provided by an Asthma Educator (in the Assignee column). Suggested providers are included for you - remove providers you don't need. Add more by clicking within the field, and selecting them from the list that appears (as shown in our example below).

       If you feel that an entire row of information does not relate to you patient, you can remove it from their Care Plan by clicking  You must be in edit-mode to see this button.

 

 

o      You can 'favourite' a condition, making it easy to select the next time you generate a Care Plan. To do so, go into edit-mode (explained above), make changes as desired (at the very least you must add a Description for the favourite), and click Favourite Template  Favourites will appear at the top of the Conditions list.

 

 

o      Additional Needs/Goals/Tasks can be inserted by scrolling to the bottom of the Condition section, and clicking

 

o      Remove an entire condition from your Care Plan by clicking located next to the name of the condition. If you cannot see this button, it is probably because you are in edit-mode.

 

 

7.      (optional) Depending on the condition you select, you may be required to include any Additional Providers to be added to your plan. This list will appear towards the bottom of the condition details.

o      The providers that appear here are replicated from those listed in the Assignee sections of your Care Plan. If there is a provider you wish to remove from this list, you will need to locate them in the Care Plan body, and remove them from there.

o      Enter an assignee for each speciality listed. This is a mandatory field.

 



8.      Now you can...

Cancel the Care Plan, and close the Care Plan panel. If what you really want to do is to return to the list of Care Plans, click instead.

Add a recall for this patient in regards to this Care Plan.

Save the Care Plan to complete at a later time.

Save the completed Care Plan to the Actions section of the Consult panel, and print it.

For the TCA and GPMP+TCA care plans, an EDC form is also added to Actions. This is a government requirement when certain specialities are selected.

 

Cycle of Care

1.      Within the patient's record, locate and click    to call the Consult panel.

2.      Within the Consult panel, locate and click  The CDM panel appears.

3.      Select Asthma or Diabetes from the Cycle of Care section. In our example, we have selected Asthma.

 

 

4.      A list of steps is displayed. When you feel a step has been completed, tick the box beside it.

5.      A recall is set for 6 months from the current date. Edit the date if desired.

6.      Click  A draft of the Cycle of Care plan is saved in the Current Cycle of Care section.

o      Click to continue working on the Cycle of Care.

o      Click to delete it.

 

 

7.      When all boxes have been ticked, click  The care plan displays as Completed in the CDM panel.

GP Mental Health Treatment Plan

1.      Within the patient's record, locate and click    to call the Consult panel.

2.      Within the Consult panel, locate and click   The CDM panel appears.

3.      Select GP Mental Health Treatment Plan. The GP Mental Health Treatment Plan form appears.

 

4.      The existing Authority Information, Assessments, Medications, Conditions, Health Summary, and Biometrics for the patient are displayed on the left-hand side.

o      Authority Information: You can select or deselect any of these options to be included in or excluded from the care plan.

o      Assessments: If no result is shown in the Assessments section, you can access the Assessments panel, and complete the listed assessment.

o      Biometrics: If you need to update this information, click

 

 

5.      On the right-hand side, enter information for each section;

o      Mental Health History/Treatment.

o      Mental Status Examination

o      Risk Assessment

o      Mental Health Treatment Plan.

o      Initial Action Plan.

 

 

6.      A recall is set for 6 months from the current date. Edit the date if desired.

 

7.      Click  A draft of the GP Mental Health Treatment Plan is saved in the GP Mental Health Treatment Plan section.

o      Click to continue working on the GP Mental Health Treatment Plan.

 

 

8.      When you have completed the form to your satisfaction, click  The status of the GP Mental Health Treatment Plan shows as Initial in the CDM panel.

9.      Later, when you are ready to review the Plan, click

o      The form opens again, but as this is a review of the initial assessment, the fields offered for input differ.

 

10.   Click to print the GP Mental Health Treatment Plan. The GP Mental Health Treatment Plan is added to the Actions section of the Consult panel.

Care Plans in the Timeline

When the consult is exited, all care plans, referral letters, and EDC forms are removed from the Actions section, and added to a new consult entry in the patient’s Timeline.

Click on a Plan to view it:

Display previous item from patient's Timeline.

Display next item from patient's Timeline.

Reprint the selected document.

Previous page (of a multi-page document).

Next page (of a multi-page document).

Fit to width.

Fit page.
